I made an early life decision to pursue medicine going to a six year medical program out of high school and then residency in orthopedic surgery. I’ve always been interested in aviation and aviation medicine. Over the past 15 years I’ve earned my private pilot license, instrument, helicopter and seaplane rating. I enjoy the process of learning and developing new skills. I look forward to meeting other pilots and helping them pursue their aspirations and careers.

Special Issuance: Additional $50
Delayed Certificate Insurance: Additional $50
Aeromedical Consultations: $350/hour
HIMS AME/IMS: $350/hour
EKG tests: $115
FAA MedXPress
Before scheduling your appointment, you must submit an FAA Online Application Process at FAA MedXPress, and complete Form 8500-8 online.
You must generate a confirmation number in order for Dr. Kostman to import your application.
A MedXPress online application is required by the FAA for all applicants.
NOTE: The MedXPress application is not official until imported and verified by Dr. Kostman. Changes to the application can be made at the time of your physical examination.
